Job description
POSITION DETAILS
* Covers 11 programs throughout the greater Philadelphia area.
* Travel required throughout Delaware, Philadelphia, Montgomery, and Chester counties.
* Typically visits one site per day.
* Regular visits to nursing homes and participant care centers.
* Participates in provider on-call rotation (typically two one-week assignments every 3–4 months).
* Supervises Primary Care Providers (PCPs) and Nurse Case Managers.
KEY RESPONSIBILITIES
* Participate in interdisciplinary team meetings and provide medical expertise.
* Conduct participant intakes and assessments.
* Develop individualized treatment and preventive care plans.
* Discuss medical conditions, prognosis, and advance care planning with participants and caregivers.
* Coordinate care across multiple healthcare settings.
* Supervise healthcare programs and collaborate on nursing treatment plans.
* Respond to emergency medical situations and provide timely interventions.
* Ensure compliance with EMR documentation standards.
* Lead culturally sensitive advance care planning discussions.
* Collaborate with specialists, consultants, and pharmacists.
* Oversee the Quality Assurance Performance Improvement (QAPI) program.
* Participate in grievance procedures involving medical care.
* Develop and maintain medical policies, procedures, and clinical protocols.
* Provide leadership and mentorship to clinical staff.
MINIMUM QUALIFICATIONS
* Current and active Pennsylvania physician license.
* CDS Certification and DEA Registration required.
* Current BLS certification required.
* Board Certified in Internal Medicine or Family Medicine required.
* Board Certification in Geriatrics preferred.
* Minimum one year of experience caring for the frail or elderly population.
* Previous supervisory experience required.
* Strong communication, leadership, organizational, and problem-solving skills.
* Proficient with Microsoft Office Suite.
* Ability to adapt to changing priorities.
* Willingness to travel throughout the service region.
