Position Overview
We are seeking a Licensed Practical Nurse (LPN) for an immediate full-time opening with a mission-driven non-profit organization operating Federally Qualified Health Centers (FQHC). This role focuses on expanding community healthcare access across primary care, behavioral health, and preventive education services.

Key Responsibilities
✔ Administer medications, treatments, immunizations, and wound care under RN supervision.
✔ Monitor, record, and report patient vital signs, clinical conditions, and recovery progress.
✔ Assist in developing and implementing care plans alongside RNs, physicians, and therapists.
✔ Educate patients and families on post-treatment care regimens, medications, and health goals.
✔ Maintain accurate clinical records, medication administration logs, and shift documentation.
✔ Uphold rigorous infection control standards, clinic safety protocols, and regulatory guidelines.
✔ Advocate for patient needs, provide compassionate support, and organize multi-patient workflows.

Qualifications
✔ Education: Certificate/Diploma from an accredited Practical Nursing program.
✔ Licensure: Active and current New York State LPN License and registration.
✔ Experience: 1–2 years of clinical experience in an ambulatory or outpatient care setting preferred.
✔ Technical Skills: Competence in venipuncture, medication administration, immunizations, and electronic health record/computer workflows.
✔ Soft Skills & Language: Excellent clinical judgment, time management, multi-patient prioritization; bilingual English/Spanish preferred.
