Home Health Physician – Michigan (Multiple Locations Statewide)

Position Overview
We are seeking Board-Certified Physicians for a flexible home health opportunity across Michigan. This role involves conducting in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. It’s an excellent opportunity for providers seeking flexibility, autonomy, and supplemental income in a non-traditional clinical setting.
Start Date
As soon as credentialed (typically 7 days to 2–3 weeks)
Schedule
Flexible scheduling, Monday–Sunday
Day shifts only: 7:00 AM – 7:00 PM
No nights or weekend requirements
Opportunities for full-time, part-time, PRN, or contract work
Practice Setting
Home Health / In-Home Evaluations
Clinical Responsibilities
Conduct 6–9 pre-scheduled in-home visits per day
Perform comprehensive evaluations including:
Medical history review
Medication reconciliation
Physical assessments
Complete patient-specific documentation using pre-populated forms
No prescribing, ordering labs, or modifying treatment plans

Board Certified (BC) – required
