Job description

Aya Locums is seeking a Primary Care NP to support flexible in-home health evaluation visits across northern California. This role is ideal for providers who want adaptable scheduling while delivering high-quality care to geriatric and adult patients in their homes.

Assignment Details
• Specialty: Primary Care NP (Geriatrics, Home Health, Hospice/Palliative, Internal Medicine, Med-Surg, Public Health, Urgent Care)
• Start Date: As soon as credentialed
• Contract Length: Flexible — short-term, long-term, full-time, part-time, PRN, travel, or seasonal
• Schedule: Flexible, Monday–Sunday, 7a–7p (no nights or weekends required)

Work Details
• Average Visits per Day: 6–9
• Patient Population: Primarily geriatric and adult
• Department Description: Pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
• Tools/Support: Pre-populated patient-specific forms provided

Requirements
• Board Certification: Required
• License: Active CA NP license
• Education: Master’s degree required
