As a patient treated by a physician provided by Virtual Medical Staff we encourage you to take part in your treatment choices, and promote your own safety by being well informed while receiving care via telemedicine.

 

PATIENT RIGHTS:

  • You have the right to receive safe and quality care within the range of services that telemedicine provides.
  • You have the right to receive considerate, respectful and compassionate care regardless of your age, gender, race, national origin, religion, sexual orientation, gender identity or disabilities.
  • You have the right to be told the names of your doctors, nurses, and all health care team members directing and/or providing your telemedicine care.
  • You have the right to have a family member, support person, or other individual be present with you for emotional support during the course of your stay, unless the individual’s presence infringes on others’ rights, safety, or is medically or therapeutically contraindicated.
  • You have the right to be told by your doctor about your diagnosis and possible prognosis, the benefits and risks of treatment, and the expected outcome of treatment, including unexpected outcomes.
  • You have the right to ask for a change of doctor, nurse or other health care team member or a second opinion.
  • You have the right to have your pain or illness assessed and to be involved in decisions about your treatment. Parent, guardians, family members or others whom you choose can speak for you if you cannot make your own decisions.
  • You have the right to have privacy and confidentiality when you are receiving care.
  • You have the right to have your medical records kept confidential and shared only with your doctor, nurse or health care team member or those who are legally authorized to view them.  You have the right to read and copy your own medical record.  You have the right to ask that your doctor change your medical record if it is incorrect, incomplete or contains irrelevant information.
  • You have the right to receive a copy of and details about your bill.
  • You have the right to know the telemedicine grievance process and share a concern or grievance about your care either orally or in writing and receive a timely notice of the resolution.  If you have a grievance or concern, please contact the Risk Manager or Director of the hospital.
  • You have the right to request an in-person consultation should you feel that the telemedicine consultation is less than adequate or otherwise unsatisfactory.
  • You have the right to ask any authorized trainees or support personnel observing or assisting in your telemedicine consultation to leave the room or discontinue viewing/listening to your consultation at your request.
  • You have the right to receive information and ask questions related to the confidentiality of your telemedicine consultation and the use of your medical information.

PATIENT RESPONSIBILITIES:

  • You are expected to provide complete and accurate information about yourself and your health, including present complaints, past health problems and hospital visits, medications you have taken and are taking (including prescriptions, over-the-counter and herbal medicines), and any other information you think your care givers need to know.
  • You should provide the hospital or your doctor with a copy of your advance directive if you have one.
  • You are expected to actively participate in your pain management plan and to keep your doctors and nurses informed of the effectiveness of your treatment.
  • You are expected to ask questions when you do not understand information or instructions.  If you believe you cannot follow through with your treatment plan, you are responsible for telling your doctor.  You are responsible for outcomes if you do not follow the care, treatment, and service plan.
  • You are expected to provide correct and complete information about your financial situation as best you can and promptly meet any financial obligations agreed to with the hospital.
  • You are expected to treat your doctors, nurses and other health care team members with respect.
